Qualifications and description for a Social Worker

  • Conduct Social Service assessments and documents according to state and federal regulations

  • Complete Psycho-Social assessment and relevant parts of Minimum Data Set (MDS)

  • Discharge Planning Experience

  • Counseling Residents

  • A minimum of a bachelor’s degree in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology. A master’s degree from a program accredited by the Council on Social Work Education is preferred.

  • Minimum of one year of supervised social work clinical experience in a health care setting collaborating directly with individuals.

  • Knowledge and ability in the use of Electronic Medical Records (EMR)

  • Demonstrate ability in oral and written communications

  • Demonstrate ability in leadership, people skills to interact, develop, implement, and sustain positive relationships with residents, families and/or authorized legal representatives, interdisciplinary professionals, other stakeholders, and provider agencies.

  • Current knowledge of available treatment options, quality indicators, infection control practices, residents’ safety issues, reimbursement regulations, billing, and receivables.
