Position summary
The primary purpose of the Social Services Director is to oversee and lead social services operations across multiple communities while ensuring residents and families receive compassionate support and guidance. This position is responsible for discharge planning, resident advocacy, regulatory compliance, supervision of social work staff, and management of Patriot Angels programs. The Social Services Director will work closely with interdisciplinary teams and leadership to ensure company goals are met and residents’ needs are addressed effectively.
Responsibilities Overview
• Provide direct counseling and social services support to residents and families.
• Coordinate complex discharge planning and follow established discharge procedures.
• Identify residents’ social, emotional, and psychosocial needs.
• Connect residents and families with appropriate community agencies and resources.
• Issue non-coverage letters and discharge notices as needed.
• Monitor accounts and collaborate with leadership regarding payment status.
• Supervise and support Social Workers throughout the organization.
• Build, develop, and manage Patriot Angels programs for all company communities.
• Review and approve Patriot Angels invoices.
• Ensure compliance with company policies and State/Federal regulations.
• Communicate consistently with residents, families, staff, vendors, and leadership.
• Support communities in North Carolina, South Carolina, Virginia, Florida, Georgia, and Alabama.
Qualifications
• Bachelor’s degree in Social Work, Human Services, Healthcare Administration, or related field preferred.
• Previous experience in social services, healthcare, senior living, or long-term care preferred.
• Leadership or supervisory experience preferred.
• Strong communication, organization, and problem-solving skills.
• Ability to maintain confidentiality and professionalism.
• Ability to multitask, prioritize, and work independently.
• Attention to detail and ability to meet deadlines.
• Proficient in Microsoft Office programs.
• Willingness to travel as needed.
