Performs minimally complex social service work/crisis services. Works under moderate supervision in a team setting, with latitude for the use of initiative and independent judgment.
Provides Crisis Services to include crisis screening & assessment, crisis follow-up, referral & linkage, and transportation to individuals with Intellectual and Developmental Disabilities or with co-occurring Mental Health and Intellectual and Developmental Disabilities. Works under moderate supervision in a team setting, with latitude for the use of initiative and independent judgment.
Examples of work performed:
- Provides Crisis Screenings and Assessments of Psychiatric needs.
- Communicates and Coordinates Services with Psychiatrist.
- Coordinates Services for In-patient Admissions for Psychiatric Services.
- Supports the MCOT team by explaining and/or providing IDD Crisis Services to Clients
- Communicates and Coordinates Services with Lead Crisis Intervention Specialist (CIS)
- Coordinates IDD Services for Clients to prevent crisis
- Provides Monitoring and Follow-up Services regarding individual needs of client.
- Provides Transportation of Clients in the Resolution of a Crisis.
- Coordination of Community Resources regarding the Specific Needs of the Client.
- Responsible for Medication Monitoring.
- Will provide consultation services on an on-call rotation.
- Resolves Crisis Incidents in the Least Restrictive Measures.
- Serves as point of contact with Mental Health and Intellectual and Developmental Disabilities departments until crisis has been managed.
- Locating, coordinating, and provision of services and supports to manage crisis and prevent further crisis.
