Open Door is currently seeking a Care Access Representative in our Ossining or Tarrytown location. The Care Access Representative has overall responsibility in a patient-centered team-based approach to promote patient access to health and wellness services. Primary responsibilities include supporting the Registered Dietitians and Behavioral Health providers with active schedule management, including patient outreach, confirmation calls, and rapid rebooking to reduce no-show rates and maximize provider productivity. In addition, the Care Access Representative will conduct Social Care Network screenings and connect patients with identified needs to follow-up navigation.
DUTIES AND RESPONSIBILITIES
- Assists in the management of the Registered Dietitian and Behavioral Health providers’ schedules to optimize access, continuity of care and productivity
- Schedule and reschedule patients
- Confirms appointments with patients via phone or text
- Assesses barriers to appointment attendance and assists patients with addressing identified barriers
- Respond to patient inquiries daily
- Conduct Social Care Network screenings and connect patients with identified needs to follow-up navigation
- Outreaches and schedules participants for participation in educational programs (e.g. group nutrition education programs)
- Manages documents and telephone encounters
- Provides excellent customer service
- Attends team and/or staff meetings as assigned
- Perform additional duties as assigned
QUALIFICATIONS
- EDUCATION: High school diploma or GED required
- Bilingual Spanish and English fluency required
- Proficient computer use, including Microsoft 365 applications
- Able to understand and follow detailed instructions
- Highly organized and self-directed, able to multi-task
- Strong interpersonal skills including excellent active listening and clear oral and written communication skills
- Strong customer service, patient-centered orientation
- Passionate about providing high quality care to low-income, under-served individuals and those lacking access to health care
- Engaging, warm and caring personality, empathetic and driven to assist patients
- Able to function effectively as a member of an interdisciplinary team and committed to collaborative team-based care
- Culturally competent/sensitive (racial, ethnic, linguistic, LGBTQ+, physical ability, etc.) and committed to inclusive, multi-cultural programming
- Flexible and solutions driven
- Comfortable working in a fast-paced, high accountability environment with different types of providers
- Effective in problem solving and exhibits excellent judgment and the ability to prioritize
- Experience providing health education and/or care/support in a medical or behavioral health setting preferred
- Maintains a safe environment of care for patients and staff
