About the Medical Assistant Position
The Medical Assistant (MA) provides clinical and administrative support to ensure the effective delivery of Chronic Care Management (CCM) services. This includes assisting with patient outreach, care coordination, health education, and documentation under the supervision of a licensed provider. The MA plays a critical role in supporting patients with multiple chronic conditions to improve outcomes and reduce hospitalizations.
Essential Duties and Responsibilities:
Chronic Care Management Support:
Identify eligible patients for CCM based on chronic condition criteria
Assist in developing and updating electronic care plans
Document non-face-to-face time spent with patients each month
Make monthly follow-up calls to:
Assess medication adherence and side effects
Review symptoms and monitor health goals
Coordinate lab appointments and referrals
Record all communications and interventions in the EHR
Ensure proper use of CPT codes 99490, 99439, or related codes
Track and report time logs for billing compliance
Clinical & Administrative Duties:
Take and record vital signs as needed
Prepare exam rooms and assist during patient visits
Update patient records, medication lists, and problem lists
Respond to patient portal messages and triage calls
Schedule preventive care and annual wellness visits
Support referrals and care coordination with specialists or community services
Compliance:
Adhere to HIPAA and confidentiality guidelines
Follow policies related to infection control and safety
Participate in training related to Medicare CCM rules and updates
Qualifications:
High school diploma or GED required
Completion of an accredited Medical Assistant program required
Certification preferred (CMA, RMA, CCMA, etc)
Familiarity with EHR systems (e.g., eClinicalWorks)
Strong communication, organizational, and time-tracking skills
