In the Clinical Care Coordinator [position or role], you will deliver integrated behavioral health support in pediatric outpatient settings, serving children ages 0–17 and their families. In this role you will collaborate with medical and behavioral health teams to coordinate care, conduct screenings, connect families to community resources, and support early intervention through the Collaborative Care Model and telehealth, with a focus on improving access and outcomes in underserved communities.
Minimum qualifications:
Minimum Education: Master’s Degree
Defined Education: Counseling, Social Work, or related Health and Human Services field
Preferences: Spanish-speaking
Substitutions: None
Years’ Experience: One (1) year
Defined Experience: Health and Human Services, mental health, or behavioral health-related field
License/Certifications:
For Master’s Degree in Counseling related field: intern status license as Licensed Professional Counselor-Associate (LPC-A), Licensed Marriage and Family Therapist-Associate (LMFT-A), and/or Licensed Master Social Worker (LMSW) in the state of Texas and actively working on obtaining professional license as LPC, LMFT, or LCSW (obtaining practice hours and required examinations); or fully licensed as LPC, LCSW or LMF; Valid Texas Driver’s License
