PRN Physical Therapist – Skilled Nursing Facility (Tyler, TX)
Position Overview
We are seeking a compassionate and skilled PRN Physical Therapist (PT) to join our team in a Skilled Nursing Facility in Tyler, Texas. In this role, you will help residents restore function, improve mobility, relieve pain, and enhance their overall quality of life. You’ll collaborate with an interdisciplinary team to deliver individualized, patient-centered care in a supportive and rewarding environment.
Key Responsibilities
- Perform comprehensive evaluations to assess patients’ physical abilities, limitations, and rehabilitation potential
- Develop, implement, and modify individualized Plan of Care (POC) based on patient needs and goals
- Deliver evidence-based therapeutic interventions to improve strength, mobility, balance, coordination, and safety
- Monitor patient progress and adjust treatment plans as needed to optimize outcomes
- Document evaluations, progress notes, physician orders, and discharge summaries accurately and timely
- Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate care
- Educate patients, families, and caregivers on treatment plans, safety, and home exercise programs
- Ensure compliance with infection control standards and safety protocols
- Participate in care planning meetings and contribute to overall patient care strategies
- Support a positive team environment through effective communication and professionalism
Qualifications
- Current, active Physical Therapist license in the state of Texas
- Minimum 1 year of experience preferred (SNF or geriatric experience a plus)
- Current CPR certification
- Strong clinical assessment and documentation skills
- Ability to work independently while collaborating effectively with a team
- Experience with HomeCare HomeBase (HCHB) or EMR systems preferred
Physical Requirements
- Ability to stand, walk, bend, squat, kneel, and reach for extended periods
- Ability to lift, transfer, and assist patients (up to 100 lbs with assistance as needed)
