At SageGrove, you’re more than your title — you’re a vital part of a team that truly values every contribution. Whether you're just getting started or looking to grow in your career, we offer a workplace where you’re supported, appreciated, and empowered to thrive as you improve the lives of others in our senior living communities.

What You’ll Do:

As our Memory Support Director, you will provide expert leadership and clinical oversight for our memory care program, directly impacting the lives of our residents and the success of our team by:

  • Implementing and managing the Portraits Program with Montessori methods and individualized components, ensuring the physical, mental, psychological, social, and spiritual needs of each memory support resident are met.
  • Conducting comprehensive resident assessments/evaluations before move-in and on an ongoing basis, updating resident service agreements as appropriate.
  • Leading, directing, and overseeing the caregiver team, ensuring high performance through effective selection, training, coaching, and performance evaluation, including ongoing dementia education.
  • Ensuring high-quality care delivery and compliance by conducting care observations, auditing documentation (including Yardi/Care Stream tasks and call light response), and implementing actions to address deficiencies.
  • Establishing and fostering positive relationships with residents, families, associates, and external partners like home health, hospice, and regulatory agencies.
  • Implementing operational processes, procedures, and best practices that ensure consistent application throughout the community, and overseeing departmental functions including budget, staffing, and record-keeping.

Requirements

What We’re Looking For:

  • 2+ years of experience and knowledge of health-related practices related to the care of persons living with dementia or cognitive impairment.
  • 1+ years of management experience including hiring staff, coaching, performance management, daily operations supervision, discipline, and counseling (preferred).
  • An associate's degree in nursing, social work, or another health-related field (preferred).
  • A Montessori Certification, or the willingness to obtain it.
  • A current CPR/BLS certification.
  • Valid driver’s license and acceptable driving record.
  • The ability to assist residents with limited mobility, lift/push/pull up to 50lbs.
  • Excellent verbal and written communication skills, with the ability to effectively collaborate with diverse groups, navigate challenging situations, and make sound decisions.
  • Strong technical skills (e.g., Word, Excel, PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates).
