Job Description

Phoenix Health Services is seeking an experienced Hospital Liaison to strengthen hospital relationships, increase qualified referrals, and support census growth for our skilled nursing facilities.

This is not a passive marketing position. We are looking for someone who understands hospital discharge planning, skilled nursing admissions, referral conversion, payer considerations, and the importance of measurable results.

Phoenix Health Services is a small, family-owned and operated senior care company. Our leadership team is hands-on, accessible, and directly involved in the daily operations of our facilities. We are looking for someone who can help referral sources understand the Phoenix difference: responsive leadership, strong staffing, quality care, clinical capability, and a personal, boutique-style approach.

Position Summary

The Hospital Liaison will be responsible for building and maintaining relationships with hospitals, discharge planners, case managers, social workers, physicians, LTACHs, assisted livings, home health agencies, and other healthcare referral sources.

The goal of this role is to increase appropriate referrals, improve hospital visibility, identify barriers to admissions, and help convert qualified referrals into successful admissions.

This position requires someone who is organized, relationship-driven, persistent, professional, and comfortable being held accountable to referral activity and census-growth results.

Key Responsibilities

Develop and maintain strong relationships with hospital case managers, discharge planners, social workers, physicians, LTACHs, assisted livings, home health agencies, and other referral partners.

Increase qualified referrals to Phoenix Health Services facilities.

Represent Phoenix professionally in hospitals, healthcare settings, and the community.

Communicate our strengths, including family-owned leadership, high-touch administration, strong staffing, quality care, therapy services, respiratory/vent capabilities where applicable, and ability to handle appropriate complex discharges.

Work closely with admissions, nursing, therapy, respiratory, business office, and facility leadership to support timely referral review and admission decisions.

Follow up on referrals, lost referrals, declined referrals, and hospital feedback.

Identify barriers that are preventing referrals or admissions and communicate those barriers to leadership.

Track referral sources, outreach activity, admissions generated, referral trends, and market feedback.

Participate in regular census/admissions meetings.

Prepare weekly reports summarizing hospital visits, referral-source contacts, referrals generated, admissions, barriers, and next steps.

Maintain knowledge of skilled nursing admissions, Medicare, Medicaid, managed care, payer requirements, and clinical capabilities.

Ideal Candidate

The ideal candidate has experience in skilled nursing, post-acute care, hospital relations, clinical liaison work, admissions, healthcare sales, or referral development.

We are especially interested in candidates who have a proven track record of growing referrals or census in a skilled nursing or post-acute setting.

Strong understanding of hospital discharge planning and post-acute placement.

Existing relationships with hospital case managers, discharge planners, social workers, physicians, or other referral sources preferred.

Knowledge of skilled nursing admissions, Medicare, Medicaid, managed care, and payer mix preferred.

Ability to speak confidently about clinical capabilities and appropriate patient placement.

Strong communication, follow-up, and organizational skills.

Ability to work independently while staying accountable to leadership.

Comfort with weekly reporting and measurable performance expectations.

Professional appearance and strong interpersonal skills.

A positive, persistent, and solutions-focused attitude.
