Purpose of Your Job Position:
The primary purpose of your job position is to plan, organize, develop and direct the overall operation of our Wellness Department in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Executive Director and the Medical Director, to ensure that the highest degree of quality care is maintained at all times.
Duties:
Director of Health Services performs initial assessment of the residents' needs for care or delegates to RN. Obtains pertinent information from family, physician, and other health providers as appropriate.
The Director of Health Services or RN designee documents findings in Resident's Wellness record and develops appropriate service or Care Plan. Communicates findings and plan to residents, his/her family (if appropriate) and facility team.
The Director of Health Services or RN designee obtains prescription for all needed medications and treatments and orders medications and supplies as appropriate.
Determines residents' capacity for self-administration of medications.
Plan, develop, organize, implement, evaluate and direct the Wellness Department, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the Assisted Living facility.
Develop, maintain, and periodically update written policies and procedures that govern the day to day functions, objectives and philosophies of the Wellness Department, including the Quality Assurance program.
The DHS completes all care plans and level of care assessments.
Follows residents when they are in hospital or sub-acute.
On call required
Communicates residents ' needs to nursing staff and care associates and directs assignment of specific tasks .
Observes residents for changes in behaviors and/or function and assesses any change reported by other staff.
Works with Executive Director and other staff in developing managed risk agreements when needed.
Education/Experience
Current NJ licensure as RN.
Bachelor's degree preferred.
At least 3 year’s full-time experience with a geriatric population.
Knowledge of State regulations
IV Therapy Certified (Preferred)
