Overall responsibility for developing new referral relationship opportunities for perspective hospice patients; acting as a resource to these accounts and existing accounts in an assigned territory.
- Graduate from an accredited School of Socia Work MSW.
- Experience in Hospice or Palliative Care preferred and experience in Account Management.
- Knowledge of COPs for Hospice and Palliative Care and reimbursement for Hospice desirable.
- Minimum of one-year experience in outreach, sales or marketing.
- CHPN after first year of hire.
- Ability to work within an interdisciplinary team environment
- Excellent public speaking abilities.
- Well-developed teaching, public speaking, leadership, and organizational skills
- Able to work creatively within budgetary constraints
- Goal Oriented Change Agent
- Excellent verbal and written communication skills
- Advanced program development skills
