Overview:

We are seeking a passionate, forward‑thinking Ambulatory Care Clinical Pharmacist to join our dynamic team. In this role, you’ll partner directly with providers, nurses, and patients to deliver high‑impact Advanced Primary Care Management (APCM) services and help shape the future of team‑based care. Additionally, you will play in integral role in the development and growth of FirstHealth Ambulatory Care Pharmacy Services.

Responsibilities:

What You’ll Do

  • Partner closely with providers to advance APCM initiatives and improve clinic‑level quality metrics
  • Provide comprehensive medication management and direct patient care under a collaborative practice agreement
  • Conduct pharmacist‑led visits for chronic disease states such as diabetes, hypertension, dyslipidemia, anticoagulation, COPD/asthma, and others
  • Use evidence‑based guidelines to adjust medications, order labs, and monitor outcomes
  • Educate patients to improve adherence, confidence, and self‑management skills
  • Document care in the EHR and contribute to value‑based care strategies


Qualifications:

What We’re Looking For

  • PharmD required; PGY1 residency preferred (PGY2 Ambulatory Care a plus)
  • Eligible for CPP (Clinical Pharmacist Practitioner) credentials in North Carolina
  • BCACP certification (or eligible) preferred
  • Strong communication and relationship‑building skills
  • Passion for primary care, chronic disease management, and patient‑centered practice
  • Desire to work in an innovative, fast‑moving outpatient environment
