What You’ll Do
• Manage a hospice caseload across homes, assisted living, and skilled nursing facilities.
• Deliver pain and symptom management focused on comfort, dignity, and quality of life.
• Serve as the primary RN and case manager for patients and families.
• Provide education, support, and guidance through end-of-life transitions.
• Collaborate with physicians, social workers, chaplains, aides, and the interdisciplinary team.
• Coordinate care to ensure consistent, high-quality patient outcomes.
• Maintain accurate, timely EMR documentation and compliance with hospice standards.
What We’re Looking For
• Active Minnesota Registered Nurse (RN) license.
• Experience in hospice, oncology, ICU, palliative care, or home health.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and relationship-building skills.
• Passion for patient-centered, end-of-life care.
• Reliable transportation and ability to travel throughout the service area.
