- Licensed Practical Nurse - Days
Schedule:
- Three (3), 12-hour shifts per week, 7:00 am - 7:30 pm. This includes an every other weekend (Saturday & Sunday) commitment.
About the Role
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Kaplan Family Hospice Residence (KFHR) Clinical Care Team to care for patients within our 11-bed residence located at 1 Sunrise Lane, Newburgh, NY 12550.This role provides patient care under the guidance of an RN, following the individualized Plan of Care. The LPN will work closely with the hospice interdisciplinary team to ensure high-quality, patient-centered care that supports patients and families through every stage of hospice care.
Essential Job Functions/Responsibilities:
- Provide direct patient care in accordance with the State Nurse Practice Act and established Plan of Care under RN supervision
Accurately document care, patient condition, and pain/symptom management
Communicate patient status changes promptly to the Registered Nurse
Deliver effective pain and symptom management and evaluate patient response
Educate patients and families on medications, treatments, diet, and self-care techniques
Collaborate with the interdisciplinary hospice team to ensure coordinated care
Adhere to infection control standards and all hospice policies and procedures
Requirements
Education Required:
- Graduate from an Accredited School of Nursing.
Experience Required:
- 1-2 years of recent medical surgical experience employed as an LPN in an institutional setting, home care, or skilled nursing facility. Previous hospice/home care experience is preferred.
What You Bring:
- Current NYS LPN Licensure.
- Basic knowledge of nursing theory and practice, knowledge of appropriate technology, knowledge of practice standards for area of responsibility, sound clinical skills, sound standards of practice, and positive communication skills.
