Job Description

Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. Hospice RN Liaison (Full-Time) – 2 Openings

Are you a compassionate RN who enjoys connecting with people and making a meaningful impact? Good Samaritan Hospice is looking for two full-time Hospice RN Liaisons to join our team!

In this role, you’ll meet with patients and families who are considering hospice care—providing clinical assessments, answering questions, and helping them navigate an important decision with confidence and comfort. You’ll also build strong relationships with hospitals, referral sources, and community providers to ensure a smooth and supportive transition into hospice services.

This is a great opportunity for a nurse who enjoys both clinical care and relationship-building. You’ll also assist with admissions when needed and play a key role in ensuring patients receive compassionate, high-quality end-of-life care.

QUALIFICATIONS FOR HOSPICE NURSE LIAISON

  • Current Registered Nurse (RN) license in the state of Pennsylvania
  • Minimum of three [3] years nursing experience
  • Marketing/Public Relations experience preferred
  • Admission and discharge planning experience preferred
  • Thorough knowledge of hospice and palliative nursing required
  • Must have the ability to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures in the hospice setting
