PAX Health LLC
- Receive, review, and respond promptly to assigned intake referrals
- Conduct biopsychosocial assessments and support diagnostic formulation under supervision
- Develop and implement individualized treatment plans with guidance from a Clinical Supervisor
- Provide direct clinical services, including individual, group, family, and/or couples therapy
- Maintain an active caseload appropriate to level of experience, with ongoing clinical oversight
- Monitor patient progress and adjust treatment approaches in collaboration with a supervisor
- Support discharge and aftercare planning to ensure continuity of care
- Provide case management services and coordinate referrals to external resources when needed
- Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR)
- Ensure coordination and integration of services documented across providers and disciplines
- Collaborate with prescribers and other members of the care team to support patient outcomes
- Participate in individual and group supervision in accordance with licensure requirements
- Attend treatment team meetings, staff meetings, and clinical trainings
- Comply with all local, state, and federal confidentiality and regulatory requirements
- Deliver culturally responsive, patient-centered care aligned with PAX Health values
- Abide by PAX Health code of ethics and model values
- Perform other duties as needed within the scope of the role
- Master’s degree in Social Work, Counseling, Psychology, or a related field
- Active and unrestricted provisional license (e.g., LSW, LAC, or equivalent)
- Working toward independent clinical licensure under appropriate supervision
- Foundational clinical skills in assessment, treatment planning, and therapeutic interventions
- Interest in developing skills in evidence-based modalities such as CBT, DBT, or similar approaches
- Experience working with diverse populations and a range of mental health conditions preferred
- Strong communication and interpersonal skills
- Strong organizational and documentation skills
- Proficiency with EHR systems
- Ability to work collaboratively within a multidisciplinary team
- Commitment to professional growth, supervision, and ongoing learning
- Commitment to ethical practice and patient-centered care
- LCADC or CADC is a plus
