Description


  • Receive, review, and respond promptly to assigned intake referrals  
  • Conduct biopsychosocial assessments and support diagnostic formulation under supervision  
  • Develop and implement individualized treatment plans with guidance from a Clinical Supervisor  
  • Provide direct clinical services, including individual, group, family, and/or couples therapy  
  • Maintain an active caseload appropriate to level of experience, with ongoing clinical oversight  
  • Monitor patient progress and adjust treatment approaches in collaboration with a supervisor  
  • Support discharge and aftercare planning to ensure continuity of care  
  • Provide case management services and coordinate referrals to external resources when needed  
  • Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR)  
  • Ensure coordination and integration of services documented across providers and disciplines  
  • Collaborate with prescribers and other members of the care team to support patient outcomes  
  • Participate in individual and group supervision in accordance with licensure requirements  
  • Attend treatment team meetings, staff meetings, and clinical trainings  
  • Comply with all local, state, and federal confidentiality and regulatory requirements  
  • Deliver culturally responsive, patient-centered care aligned with PAX Health values  
  • Abide by PAX Health code of ethics and model values  
  • Perform other duties as needed within the scope of the role 
Requirements
  • Master’s degree in Social Work, Counseling, Psychology, or a related field  
  • Active and unrestricted provisional license (e.g., LSW, LAC, or equivalent)  
  • Working toward independent clinical licensure under appropriate supervision  
  • Foundational clinical skills in assessment, treatment planning, and therapeutic interventions  
  • Interest in developing skills in evidence-based modalities such as CBT, DBT, or similar approaches  
  • Experience working with diverse populations and a range of mental health conditions preferred  
  • Strong communication and interpersonal skills  
  • Strong organizational and documentation skills  
  • Proficiency with EHR systems  
  • Ability to work collaboratively within a multidisciplinary team  
  • Commitment to professional growth, supervision, and ongoing learning  
  • Commitment to ethical practice and patient-centered care  
  • LCADC or CADC is a plus
