Centralized Admissions Coordinator **Skilled Nursing Facility**  – Join Our Growing Team! ?

 

We’re looking for a motivated, detail-oriented, and compassionate individual to join our Centralized Admissions Team, supporting hospital partnerships and helping patients access the care they need.

Position: Centralized Admissions Coordinator

Location: Primary location in Avon, CT. Must be willing to travel as needed.

Type:

  • Full-Time Monday-Friday

  • Rotating holidays and weekends

  • Per Diem weekends

     

What You’ll Do:

  • Review referrals from hospitals for all our skilled nursing facilities

  • Collaborate with hospital case managers and internal teams to ensure smooth, timely admissions

  • Make clinical and financial admission decisions that help patients transition to the right level of care

  • Plan, organize, and execute the admission process, ensuring compliance with all regulations and guidelines.

  • Be part of a high-performing, fast-paced team that values accuracy, critical thinking, and a patient-first approach

     

What We’re Looking For:

  • Strong communication and problem-solving skills

  • Strong attention to detail, follow-through skills, and ability to prioritize tasks.

  • Ability to assess clinical information and make sound admission decisions

  • Knowledgeable of admissions and discharge procedures, current laws, residents’ rights, and documentation requirements

  • Experience in healthcare, case management, admissions, or related field preferred

  • Someone who thrives in a team environment and is ready to grow with us

  • Ability to remain calm in stressful situations and communicate professionally
