Job Description

Our behavioral health organization is dedicated to supporting the emotional and psychological wellness of older adults across New York State. We specialize in delivering compassionate psychiatric services to residents in long-term care and post-acute rehabilitation environments.

We are currently expanding our geriatric behavioral health team and are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) who shares our commitment to exceptional patient-centered care. Requirements

Key Responsibilities

  • Perform comprehensive psychiatric assessments and develop individualized treatment plans.

  • Prescribe, manage, and adjust psychotropic medications in collaboration with attending providers.

  • Provide training and educational sessions for facility staff on behavioral health best practices.

  • Work closely with nursing, therapy, and social work teams to ensure coordinated, holistic care.

  • Maintain accurate, compliant, and timely documentation in the electronic health record (EHR).

  • Participate in interdisciplinary meetings, case reviews, and quality improvement initiatives.


Qualifications

  • Active New York State Nurse Practitioner license and current PMHNP certification.

  • At least one year of psychiatric or behavioral health experience is preferred (new graduates encouraged to apply).

  • Proficient understanding of psychopharmacology and evidence-based interventions.

  • Strong clinical judgment, diagnostic, and communication skills.

  • Empathetic, patient-focused approach to care delivery.

Comfortable working with EHR platforms and telehealth technology.
