Responsibilities
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor and document patient conditions and progress
- Provide wound care, catheter care, and other skilled nursing services
- Educate patients and family members on health conditions and care plans
- Coordinate care with physicians, therapists, and other healthcare professionals
- Maintain accurate and timely documentation in compliance with agency and state regulations
Qualifications
- Active RN or LPN license in the state of Indiana
- Minimum 1 year of nursing experience (home health experience preferred)
- Current CPR certification
- Valid driver’s license and reliable transportation
- Strong clinical assessment and documentation skills
- Excellent communication and interpersonal skills
- Ability to work independently
