Job Description
The YMCA is seeking a compassionate and self-motivated Community Health Worker to build trusting relationships, support client well-being, and connect individuals to essential resources throughout the Greater Twin Cities Metro Area. 
 
 

The salary range for this position is $20.29 - $27.05.  We take into consideration an individual’s background and experience in determining salary; therefore, base pay offered may vary considerably depending on job-related knowledge, skills, and experience.  The compensation package includes a wide range of other benefits and earned time off that vary depending on part-time or full-time status.

 

1. Job Summary: 

A Community Health Worker (CHW) serves as a bridge between individuals, communities, and healthcare or social service systems to improve overall well-being, bridge gaps between healthcare systems and underserved communities, improving access and health outcomes.  This role is grant funded and is contingent upon continuous funding. 

Working hours: 8:30am-5:00pm, Monday-Friday  

 

2. Essential Functions:

  • Conduct in-person visits in clinic, hospital or home to assess needs, provide support, and ensure access to care.
  • Build trusting relationships with individuals and communities to promote and motivate health and well-being.
  • Connect clients to essential services such as food, housing, transportation, and healthcare or working out with them at the YMCA
  • Collaborate with healthcare providers, social workers, and community organizations to support clients holistically.
  • Maintain accurate records of client interactions, referrals, and progress.
  • Complete required trainings and certifications timely and stay updated on best practices.
  • Identify barriers to care and help clients navigate challenges using available resources.
  • Attend all required meetings including internal YMCA meetings and external community-based meetings.
  • Serve as a good steward of the resources available. 

 

3. Qualifications: 

Required

  • High School degree or equivalent.
  • CPR, First Aid, AED certified, or ability to obtain within first 30 days of employment.
  • Minimum one (1) year experience in a customer-facing. role of any type.
  • Knowledge of the participants’ community
  • Demonstrated proficiency in both time management practices and clear, effective communication.
  • Ability to effectively problem solve utilizing all resources available.
  • Must be able to work in an independent and self-directed manner
  • Experience working with diverse populations
  • Proficient in Microsoft Office, including Word, Excel and Outlook. 

Preferred

  • Multilingual  

 

4. Work Conditions:

  • Ability to perform a variety of physical activities with program participants which may include but are not limited to walking, standing for long periods of time, running, throwing and lifting up to 50lbs.
  • Ability to travel locally to various Y locations to attend meetings.
  • Ability to travel to local clinics, hospitals or participant homes. 

5. Relationships: 

This position reports to the Program Executive who reports to Vice President of Community Impact. The incumbent interacts regularly with youth, families, community partners/agencies, educators, volunteers and other YMCA program staff.
