As the Home Health RN Clinical Manager, you will lead and elevate the delivery of high-quality care in a home health setting. This role is ideal for a dynamic nursing leader who thrives on building strong teams, driving clinical excellence, and ensuring patients receive compassionate, safe, and effective care—right where they need it most.

You’ll serve as a key connector between clinical teams, patients, and leadership, ensuring seamless care coordination, regulatory compliance, and continuous quality improvement.

What You’ll Do

• Lead Clinical Operations: Oversee day-to-day clinical functions to ensure quality, safety, and continuity of care in compliance with federal and state regulations (Conditions of Participation).
• Drive Quality & Outcomes: Champion performance improvement through QAPI initiatives, identifying opportunities and implementing corrective actions.
• Develop & Support Teams: Supervise, coach, and mentor clinical staff to ensure adherence to care plans, strong patient outcomes, and professional growth.
• Enhance Care Coordination: Collaborate with interdisciplinary teams to ensure comprehensive, patient-centered care planning and execution.
• Ensure Compliance: Maintain regulatory excellence while supporting staff education on payer requirements and best practices.
• Promote Patient Safety: Lead infection control practices and ensure adherence to safety protocols across all care activities.
• Communicate with Impact: Foster a culture of accountability, teamwork, and clear communication across all levels of the organization.

What You Bring

• Current Registered Nurse (RN) License
• 2+ years of Home Health management experience
• Strong knowledge of home health regulations and compliance
• Experience with HomeCare HomeBase (preferred)
• Current CPR Certification
• Valid Driver’s License and Auto Insurance
