PRN Hospice Chaplain
The Hospice Chaplain is a compassionate and dedicated professional with a true calling to serve patients and families facing end-of-life care. This role is responsible for assessing and addressing the spiritual needs of terminally ill patients and their loved ones in the home setting. The Chaplain serves as a key member of the interdisciplinary team, ensuring that each patient receives holistic, patient-centered care that honors their beliefs, values, and preferences.
Key Responsibilities
- Assess patients and family support systems to identify spiritual needs, as reflected in the Plan of Care (POC), documentation, clinical records, and IDG conference discussions
- Provide ongoing spiritual and emotional support to patients and their families throughout the hospice journey
- Facilitate visits from clergy, pastoral counselors, or other spiritual advisors as requested or appropriate
- Participate actively in interdisciplinary group (IDG) meetings and contribute to collaborative care planning
- Maintain accurate, timely, and compliant documentation of all spiritual care interventions
- Foster effective communication and interpersonal relationships with patients, families, and team members
- Support and promote the agency’s philosophy, mission, and standards of care
- Ensure continuity and coordination of care through collaboration with the interdisciplinary team
- Maintain current knowledge of hospice regulations, eligibility standards, and best practices
- Complete all required continuing education and training
Qualifications
- Minimum of one (1) year of experience as a Chaplain in a healthcare setting (preferred)
- At least one (1) year of hospice experience (required)
- Current CPR Certification
- Valid Driver’s License and reliable transportation with active auto insurance
- Experience with HomeCare HomeBase (preferred)
