PRIMARY DUTY
The primary purpose of this position is to provide safe and timely transportation of residents and staff to and from designated locations following established safety and transportation procedures.
EXPECTATIONS OF EMPLOYEE
- Observe all facility safety policies and procedures.
- Complies with state, federal, and all other applicable health care and safety standards
- Follow facility Residents� Rights policies.
- Protect the confidentiality of all resident information.
- Assists families and other visitors as needed.
- Attends/completes required in-services and other required meetings.
- Come to work in clean, neat attire and consistently present a professional appearance.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Assume accountability for data contained in the employees� handbook.
- Perform all duties assigned in an effective, timely and professional manner.
- Accept assigned duties in a cooperative manner.
- Perform other related duties as directed by his/her supervisor.
ESSENTIAL JOB FUNCTIONS include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Transport residents, staff, volunteers, and others to scheduled events
- Coordinate transportation services with appropriate staff members
- Assists residents in and out of the vehicle, using proper safety procedures
- Operates mechanical lift as required for wheelchair bound patients
- Complies with traffic regulations to operate vehicle in a safe and courteous manner
- Follows written and verbal geographic directions (such as maps or GPS directions)
- Responsible for the timely arrival of residents to and from appointments outside of the facility (flexible work hours as appropriate to appointment requirements)
- Assist the Activity Director in implementing activity programs to meet residents� needs
- Assist Activity Directory with required recordkeeping
- Observe infection control procedures related to the Activity Program, including identification of residents not appropriate to attend out-of-the-room activity programs for health reasons.
- Responsible for the safety and personal care of residents under his/her supervision.
- Assume Quality Assurance duties as assigned.
- Monitor and report necessary mechanical maintenance needed to ensure safe operation of vehicle, including reporting of any mechanical problems encountered with vehicles
- Inspect and maintain vehicle supplies and equipment, such as gas, oil, water, tires, lights and brakes, to ensure that vehicles are in proper working condition
- Maintain records such as vehicle logs and destinations as required
- Keep vehicle interior clean; remove trash and sweep as needed
- Report any issues or problems that may arise to the Administrator
EDUCATION and EXPERIENCE an equivalent combination of education, training and experience will be considered.
- High school diploma or equivalent and one year of commercial driving experience is preferred
- Must maintain current and valid State of Tennessee driver�s license; CDL Class �F� or higher as designated by the State
SUPERVISORY REQUIREMENTS of this position are generally as follows:
- This position does not have supervisory responsibility

- Operation Monitoring � Watching gauges, dials, or other indicators to make sure a machine is working properly
- Control Precision � The ability to quickly and repeatedly adjust the controls of a machine or a vehicle to exact positions
- Organizational Support � Follows policies and procedures; completes tasks correctly and on time
- Adaptability � Adapts to changes in the work environment; changes approach or method to best fit the situation; Able to deal with frequent change, delays, or unexpected events
- Attendance/Punctuality � Ensures work responsibilities are covered when absent; arrives at meetings and appointments on time

COMPUTER SKILLS/EQUIPMENT USED TO PERFORM THE JOB which may be representative but not all inclusive of those commonly associated with this position.
- Handicap accessible motor vehicles, including wheelchair lifts and emergency exit doors
- Knowledgeable of federal, state and local traffic laws and regulations governing the operation of a commercial fan for passenger use
- Basic computer skills with the ability to create forms, spreadsheets, schedules and other documents
WORK ENVIRONMENT environmental or atmospheric conditions commonly associated with the performance of the functions of this job.
- The employee is occasionally required to wear protective clothing.
- The employee may be exposed to bodily fluids and odors, dust or fumes.
- The noise level in the work environment is usually moderate.

- While performing the duties of this job, the employee is frequently required to drive a motor vehicle, talk or hear, stand, walk, sit, stoop, kneel or crouch, use hands to use hands to grasp, manipulate or feel objects and reach with hands and arms.
- The employee must regularly lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
- Specific vision abilities required by this job include close vision, distance vision, color discrimination, peripheral vision, depth perception, and the ability to adjust focus.
Clean Background check and drug screen� Long term care and activities experience preferred� Must have good communication skills and the ability to interact well with others.
- QUALIFICATIONS� High School Diploma or equivalent preferred�
- Class F driving endorsement.
