Designated Coordinator / LPN (Combination Administrative Role)


We are seeking a dedicated and experienced Designated Coordinator / Licensed Practical Nurse (LPN) to join our team in a group home setting. This is a combination administrative and direct care role, ideal for someone who is organized, supportive, and passionate about leading a team while ensuring high-quality care for individuals served.



Schedule:

  • Wednesday – Sunday
  • 2:00 PM – 10:00 PM


Position Overview:

This role blends leadership, compliance, and hands-on support. You will oversee daily operations, support direct care staff, and ensure all MN DHS 245D requirements are met while also assisting with nursing-related duties.


Key Responsibilities:

  • Conduct and oversee MAR (Medication Administration Record) audits
  • Complete and manage SComms (Staff Communications)
  • Provide supervision and support to direct care staff
  • Assist with completion of LPN duties as needed during shifts
  • Monitor and review ISP data and documentation
  • Track and ensure progress on individual goals
  • Provide and coordinate staff training, including:
    • Medication administration training
    • CPR training
  • Perform medication administration as required
  • Serve as a resource for staff by answering questions and providing guidance
  • Help maintain compliance with MN DHS 245D regulations
  • Step in to fill open shifts as needed


Qualifications:

  • Current LPN license (Minnesota) required
  • Experience in group home or 245D services preferred
  • Strong leadership, organizational, and communication skills
  • Ability to multitask between administrative and direct care responsibilities
  • Experience with staff training and supervision is a plus
  • Compassionate, dependable, and team-oriented
