Job Summary
We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to provide high‑quality direct patient care in a home health setting. The LVN will work under the supervision of a Registered Nurse (RN) to implement individualized plans of care, deliver nursing treatments, and support positive patient outcomes. This role is ideal for nurses who enjoy independent work, collaboration, and patient-centered care.
Key Responsibilities
Provide direct patient care in accordance with the State Nurse Practice Act
Implement and follow the plan of care as directed by the Registered Nurse
Accurately document patient care and clinical observations in a timely manner
Collect assessment data and communicate changes in patient condition to RNs and physicians
Collaborate with the interdisciplinary care team
Assist with coordination of home health services by reporting patient needs
Qualifications & Requirements
Active California LVN license
1+ years of nursing experience, home health preferre
Knowledge of and compliance with professional nursing standards
Strong verbal and written communication skills
Excellent organization, documentation, and time management abilities
CPR certification (current and maintained)
