Job Summary

We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to provide high‑quality direct patient care in a home health setting. The LVN will work under the supervision of a Registered Nurse (RN) to implement individualized plans of care, deliver nursing treatments, and support positive patient outcomes. This role is ideal for nurses who enjoy independent work, collaboration, and patient-centered care.

Key Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act

• Implement and follow the plan of care as directed by the Registered Nurse

• Accurately document patient care and clinical observations in a timely manner

• Collect assessment data and communicate changes in patient condition to RNs and physicians

• Collaborate with the interdisciplinary care team

• Assist with coordination of home health services by reporting patient needs

Qualifications & Requirements

• Active California LVN license

• 1+ years of nursing experience, home health preferre

• Knowledge of and compliance with professional nursing standards

• Strong verbal and written communication skills

• Excellent organization, documentation, and time management abilities

• CPR certification (current and maintained)
