The Patient Safety and Quality Coordinator is a healthcare professional who works to ensure that patients receive safe and quality care. They are driven to improve the quality of healthcare delivered to patients by reducing errors and preventing injuries. The coordinator role will monitor and respond to patient safety events, submit reports to regulatory agencies as required, debrief and support team members after safety events, analyze relevant data to identify gaps and reduce the likelihood of errors, help leaders conduct risk assessments, review and assist with updating relevant organizational policies, educate care team members about patient safety, and promote a culture of safety across the organization. The Patient Safety and Quality Coordinator will facilitate QIC (Quality Improvement Committee) meetings, root cause analysis meetings, action planning meetings, and Peer Review.


PRIMARY DUTIES AND RESPONSIBILITIES

  • Provides staff education to nurses, physicians, health staff, other clinical departments, committees, and others regarding concepts of patient safety and quality outcomes. Assists with dissemination of safety, quality, and performance improvement information. Including areas of opportunity and successes.
  • Participates in clinical rounding to engage with caregivers and frontline staff responsible for meeting safety and quality standards.
  • Maintains working knowledge of emerging safety and quality measurement and benchmarking initiatives internal to the organization and in the industry.
  • Conducts root cause analysis investigations and develop corrective action plans.
  • Serve as a liaison between the facility and The Joint Commission, state survey agencies, and other external regulatory bodies.
  • Reviews, investigates, and analyzes patient safety events. Provides recommendations for interventions to improve or enhance safety and quality based on analysis. Works collaboratively with key stakeholders to organize cross-service quality teams to review processes and methodologies, analyze results and implement plans for quality improvement.
  • Manages and assists with performance improvement projects. Assists in establishing operational performance measurements and monitoring of key performance indicators.
  • Reviews available quality and patient safety data to identify areas of high risk for deviation from standards and assist with the development of quality improvement plans. Assesses the need for internal audits of patient safety and quality processes and metrics, evaluates audits and provides feedback for improvement.
  • Assists in the development and implementation of clinical policies/procedures and clinical regulatory plans related to patient safety and quality improvement.
  • Supports and facilitates the physician peer review (FPPE/OPPE) processes.
  • Perform other duties as assigned.

ESSENTIAL QUALIFICATIONS

  • Active RN License
  • Five to ten years of relevant work experience in a complex healthcare organization.
  • At least one year of Leadership experience.
  • Strong critical thinking skills, analytic skill, and great attention to detail.
  • Exceptional organizational and interpersonal skills.
  • Provides clear concise written and verbal communication skills, objective judgement, self-direction and motivation.
  • Ability to function effectively with a wide variety of disciplines, including physicians and nurses.
  • Extreme degree of professionalism, discretion, and confidentiality.
  • Comfort interacting with and publicly presenting to a broad array of administrative and clinical staff, including senior management.
  • Collaborative and team oriented.
  • Professional confidence is essential as the role is largely independent and self-sustaining with key involvement as a process and change leader.
  • Strong project management aptitude with experience in planning, facilitating, and organizing improvement teams.

PREFERRED QUALIFICATIONS

  • Bachelor of Science degree in Nursing or health care related field
  • Certified Professional in Healthcare Quality (QPHQ) certification
