Social Worker

Job Summary:

Deer Meadows Rehabilitation Center (DMRC) is seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will play a vital role in providing emotional, social, and psychological support to our residents and their families. Social Worker Responsibilities:

  • Provide social services to residents, including counseling, therapy, and support groups

  • Conduct assessments and develop individualized plans to meet the unique needs of each resident

  • Facilitate communication and collaboration between residents, families, and healthcare professionals

  • Identify and address psychosocial and emotional needs of residents, and develop strategies to improve their overall well-being

  • Assist residents in adjusting to new living environments and coping with loss, change, and transitions

  • Participate in interdisciplinary team meetings to develop comprehensive care plans

  • Maintain accurate and up-to-date records of resident services and progress

Social Worker Requirements:

  • Bachelor's degree in Social Work or a related field

  • Minimum 1 year experience in social work position or working with geriatric population

  • Strong communication, interpersonal, and problem-solving skills

  • Ability to work effectively in a fast-paced environment and prioritize tasks accordingly

  • Compassion, empathy, and understanding of the needs of seniors and their families
