Job Description

Job Title: Clinical Progression Facilitator

Position Summary

The Clinical Progression Coordinator is responsible for facilitating the efficient progression of patients throughout the continuum of care. This role works collaboratively with interdisciplinary teams to ensure timely care delivery, reduce delays, and support safe and effective patient throughput from admission to discharge.


Key Responsibilities

  • Monitor and track patient progression from admission through discharge to ensure timely movement across the continuum of care

  • Collaborate with physicians, nursing staff, case management, and ancillary departments to coordinate care and remove barriers

  • Identify and escalate delays in care, including pending diagnostics, consults, or discharge obstacles

  • Participate in daily interdisciplinary rounds to support care planning and progression goals

  • Assist with discharge planning to ensure appropriate level of care and timely transitions

  • Maintain accurate documentation and track key performance indicators such as length of stay and avoidable delays

  • Support process improvement initiatives aimed at enhancing patient flow and operational efficiency

  • Communicate effectively with patients, families, and care teams regarding care progression when appropriate


Qualifications

  • Bachelor’s degree in Nursing, Healthcare Administration, or a related field preferred

  • Active clinical license (RN, LPN, or allied health professional) preferred

  • Minimum of 2–3 years of clinical or healthcare operations experience

  • BLS/ACLS from AHA only
  • Experience in case management, utilization review, or patient flow strongly preferred

  • Knowledge of healthcare regulations, discharge planning, and care coordination processes
