Job Details Overview: As a Licensed Practical Nurse (LPN), you will provide skilled nursing services in accordance with physician orders and agency standards. You will play a key role in delivering high-quality, patient-centered care in the home setting working under the supervision of a Registered Nurse. Responsibilities include implementing individualized care plans, administering treatments, monitoring patient conditions, and supporting care coordination. You will also educate and support patients and their families to promote health, safety, and independence at home.

Responsibilities:

Assessment & Care Planning

  • Perform ongoing assessments of patients’ physical, emotional, and environmental status

  • Assist in developing and implementing individualized care plans in collaboration with the RN and care team

  • Report changes in patient condition promptly

    Medication Administration

  • Administer medications and treatments as prescribed

  • Monitor patients for adverse reactions and therapeutic effectiveness

  • Ensure proper medication storage and compliance with safety standards

    Wound Care & Clinical Interventions

  • Provide wound care, including dressing changes and infection prevention

  • Monitor healing progress and report concerns to supervising RN/physician

  • Perform other skilled nursing tasks within LPN scope of practice

    Patient & Family Education

  • Educate patients and caregivers on disease processes, medications, and home care techniques

  • Promote independence and self-management strategies

    Monitoring & Evaluation

  • Observe and document patient responses to care

  • Communicate updates and collaborate on care plan adjustments

    Care Coordination & Collaboration

  • Work closely with RNs, physicians, therapists, social workers, and home health aides

  • Support continuity and quality of care across disciplines

    Documentation & Compliance

  • Maintain accurate, timely, and compliant clinical documentation

  • Adhere to agency policies, state regulations, and federal guidelines

    Patient Advocacy & Support

  • Provide compassionate care and emotional support

  • Respect patient rights, cultural preferences, and privacy

Qualifications:

  • Current Licensed Practical Nurse (LPN) license in the state of New York

  • Graduate of an accredited practical nursing program

  • Minimum 1 year of clinical experience; home care experience preferred

  • Strong clinical, communication, and organizational skills

  • Ability to work independently in a home setting

  • Proficiency with electronic medical records (EMR) and basic computer applications (e.g., Microsoft Word, Excel)

  • Valid driver’s license and reliable transportation

Job type: Full-time

Pay: $70,000.00-$75,000.00
