What You Will Do
- Provide direct resident care services, including medication administration and assistance
- Follow individualized service plans (ISP) for all residents
- Perform resident assessments in accordance with state regulations and community policies under the direction of the Director of Assisted Living
- Observe residents for changes in condition and report concerns promptly to the supervisor and/or Executive Director
- Provide timely interventions for changes in condition and communicate updates to care staff and families as appropriate
- Accurately document all observations, incidents, interventions, and medication assistance throughout the shift
- Communicate effectively with physicians, families, and team members as needed to support quality care
- Ensure proper shift-to-shift communication to maintain continuity of care
- Support and oversee timely documentation by Resident Assistants, including services provided and any noted changes
- Maintain and update individualized care plans for each resident
- Perform other duties as assigned by the supervisor
Why You Are Qualified
- High school diploma or equivalent (GED) required
- Minimum of one (1) year of related experience preferred
- Medication Technician certification (as required by state regulations)
- Strong attention to detail and ability to follow medication administration protocols
- Ability to perform physical tasks including standing, walking, bending, and assisting residents throughout the shift
- Must be able to lift up to 25 pounds unassisted
