What You Will Do

  • Provide direct resident care services, including medication administration and assistance
  • Follow individualized service plans (ISP) for all residents
  • Perform resident assessments in accordance with state regulations and community policies under the direction of the Director of Assisted Living
  • Observe residents for changes in condition and report concerns promptly to the supervisor and/or Executive Director
  • Provide timely interventions for changes in condition and communicate updates to care staff and families as appropriate
  • Accurately document all observations, incidents, interventions, and medication assistance throughout the shift
  • Communicate effectively with physicians, families, and team members as needed to support quality care
  • Ensure proper shift-to-shift communication to maintain continuity of care
  • Support and oversee timely documentation by Resident Assistants, including services provided and any noted changes
  • Maintain and update individualized care plans for each resident
  • Perform other duties as assigned by the supervisor

Why You Are Qualified

  • High school diploma or equivalent (GED) required
  • Minimum of one (1) year of related experience preferred
  • Medication Technician certification (as required by state regulations)
  • Strong attention to detail and ability to follow medication administration protocols
  • Ability to perform physical tasks including standing, walking, bending, and assisting residents throughout the shift
  • Must be able to lift up to 25 pounds unassisted
