Hamilton Grove Healthcare and Rehabilitation is currently seeking LPN's to join our nursing team. Our ideal candidate will have prior experience in a long term care / skilled nursing facility setting. Responsibilities include but not limited to are:

• Provide nursing care according to physician orders in accordance with recognized nursing techniques and procedures
• Administer medication and treatments per physician orders
• order medications from pharmacy, maintain narcotic records
• Perform shift to shift report and medication counts
• Assess residents upon admission and initiates the nursing care plan
• Identifies residents risks and initiates preventative care
• Schedules appointments and arranges for transportation for residents and communicating arrangements to residents and responsible parties

Other duties as assigned.

Full Times Rates Offered 32.00-40.00 (depending on Shift, Experience & Frills Option)

$5.00 Weekend Differential offered to Per Diems
