We are seeking an experienced RN Case Manager to oversee care coordination, ensure compliance, and support clinical excellence across our growing Northern New Jersey service area.
Assumes responsibility for individual medical case management of those served by Guided Steps NJ on a case-by-case basis. Responsible for ensuring the quality of medical care provided meets DDD and Agency standards while working cooperatively with the Clinical Director and Executive Director to uphold agency and licensing regulations.
The RN case manager will be responsible for:
· Monitor overall health status of participants through regular home visits and medical documentation review – weighing blood pressure, etc.
· Create applicable policies and procedures relating medical needs of the individuals, the departments and agency
· Interpret medical reports.
· Reports to a physical site to review all the in house medical records. Ensures the timely completion of medical follow up and annual examinations and provides recommendations for care. Records progress notes at sites
· Provide general nursing care/treatment to participants as ordered /prescribed, including medication administration, wound dressing, bandaging and/or other special procedures etc.
· Reviews and prepares all individual incident reports and provides medical input where indicated.
· Ability to determine the needs and abilities of the individuals and solve problems.
· Cooperates fully with licensee Agency and Department of Human Services and DDD staff in any inspection or investigation.
· Provide on-going specialized training such as but not limited to Seizure Intervention, Diabetes Management, Positioning and Lifting, Administration of Injectable Medication, Proper Use of a Nebulizer etc.
· Attends all mandatory Agency training and other training as is identified by the Executive Director.
· Communicates and collaborates with other team members and community agencies in planning, implementing and coordination and evaluating all services necessary to insure comprehensive care.
· Assesses the educational needs of the staff and conducts or arranges for appropriate in-service training programs. Serve as medical liaison as needed with family members, professionals and other individuals/organizations in the community as needed.
· Provide evaluations of individual’s health status and recommend treatment when necessary.
· Monitor medication administration and documentation in the programs to ensure quality of supports. Prepare detailed analysis of medication monitoring.
· Maintains the confidentiality, security and integrity for the management of individual’s information according to agency and DDD policies
· Attend ISP and IDT meetings as necessary as an advocate for the medical needs of the participant.
· Perform other duties as required by the nature of the position or as assigned.
Requirements:
· BSN degree (Required)
· RN License (Required)
· 2+ years’ experience in the nursing field. (Required)
2+ years’ experience working with individuals with special needs (preferred)
· Valid Driver’s License (Required)
· Clear all State/Federal Criminal Background checks, Central Registry, drug tests as required by NJ-DDD
