Description

WinnMed is looking to add a 0.8 FTE (32 hours per week) RN position in our Home Health and Hospice department!

This RN's primary responsibility is to provide care for Home Health and Hospice patients in collaboration with the patient, family, and health care providers.  This position will travel to the home or facility where the patient is residing. This position works Monday - Friday days with rotating evening, weekend and holiday call.

Requirements

• Active license to practice as an RN in Iowa required
• American Heart Association BLS required
• Current drivers license required
• Excellent interpersonal skills with a focus on patient centered care
• EHR and computer skills 
• Ability to work independently and as a team player
• Prior experience in home health and/or hospice beneficial
