POSITION SUMMARY:
The Certified Diabetes Educator assists the clinical team with diabetes-specific education and care for patients, providing nutrition education and counseling in both individual and group settings, and working as part of a clinical team to provide coordinated care (medical, educational, and social services).
DUTIES AND RESPONSIBILITIES:
Essential Functions:
Fields clinical nutrition and diabetes questions from staff.Assists Diabetes Program Manager with standardized DSMES program implementation.
Evaluates and monitors the nutritional status of patients living with diabetes.
Conducts Diabetes-related nutrition education and counseling sessions.
Organizes and develops nutritional care plans and individualized therapeutic diets as needed.
Completes necessary reports and forms and document patient services in the appropriate medical record.
Facilitates related services for health center clients as appropriate with respect to their confidentiality and privacy.
Organizes, reviews, and distributes Diabetes education materials and patient screening forms.
Serves as a resource and educator of diabetes care to the clinical and social service staff.
Stay current on the latest diabetes management techniques and treatments.
Attends conferences and participates in trainings/informal sessions as requested by the Director of Nutrition.
Works closely with providers on treatment adherence, case management, and social service staff, to improve patient services.
Conferences with the Director of Nutrition/Diabetes Program Manager as necessary to seek consultation and share information that is pertinent to patient care.
Participates in clinic and central office team meetings as designated.
Plans and facilitates educational presentations and workshops for the agency.
Compliance with Employee Health Services.
Participates in the development of Diabetes-related program policies.
Perform other related duties as assigned by the Director of Nutrition.
EDUCATION & EXPERIENCE REQUIRED:
- Bachelor’s Degree in Nutrition required, Master's preferred
- Registered Dietitian/Certified Dietitian-Nutritionist AND Certified Diabetes Care and Education Specialist License Required
- Minimum of two (2) years of nutritional experience in a clinical/community health setting
- Demonstrated experience and/or training in Diabetes Education and/or Care and Medical Nutrition Therapy
