Your Responsibilities:

• Assist clients with personal care needs such as bathing, grooming, toileting, and dressing

• Help with meal preparation, light housekeeping, and medication reminders

• Monitor client safety and report any changes in condition

• Accurately document visit notes using EVV

• Follow care plans developed by our care coordination team

• Provide companionship and emotional support

• Adhere to infection control and safety protocols

• Arrive on time and communicate clearly with the office and clients

Qualifications:

• Must be 18 years or older

• Minimum 2 years of home care experience preferred

• Must have reliable transportation

• Ability to read, write, and communicate effectively in English

• High school diploma or equivalent

• Must be professional, dependable, and able to work independently

Work Location:
Client homes throughout Philadelphia, PA 19124 and nearby areas
This is an in-person role with travel to client residences.
