Physical Therapist Assistant – Home Health
Location: Ocean County, NJ
VNA Health Group is seeking a compassionate and skilled Physical Therapist Assistant (PTA) to provide high‑quality, one‑on‑one rehabilitative care to patients in their homes. Under the supervision of a Licensed Physical Therapist, the PTA is responsible for implementing individualized treatment plans that enhance mobility, restore function, and promote independence.
This role offers the unique benefits of home care—independent practice, personalized patient interaction, and the ability to treat patients directly in their preferred environment. PTAs typically provide 6–7 patient treatments per day and serve adults ages 21 and up with diagnoses including orthopedic, neurological, cardiac, and general debility. Clinicians have immediate access to supervisory support while in the field, as well as a comprehensive, hands‑on Home Care Orientation program.

What You Will Do
Provide physical therapy treatments under the direction and supervision of a Licensed Physical Therapist
Follow the established plan of care and adjust treatment strategies based on patient progress
Accurately document treatments, observations, and patient responses in the EMR
Travel to and from patient homes throughout the day
Participate in QAPI initiatives, in‑service trainings, and organizational development activities
Adhere to infection control standards, universal precautions, and safety policies
Demonstrate professionalism, reliability, and effective communication with patients, caregivers, and colleagues
What You Will Need
Associate’s degree from a CAPTE-accredited Physical Therapist Assistant program
Active New Jersey PTA license
Minimum of 6 months PTA experience; 1–2+ years preferred
Home care experience preferred
CPR certification
Valid driver’s license and reliable transportation
Strong organizational, communication, and time‑management skills
Ability to work independently with ongoing support and collaboration from supervising PTs
Ability to maintain confidentiality and handle sensitive information appropriately
Working Conditions
Field Environment
Travel between patient homes by car
Exposure to changing weather conditions, including inclement weather
Varied home environments based on caseload
Possible exposure to bodily fluids, waste, and bloodborne pathogens (PPE required)
Possible exposure to infectious diseases and pathogens
Occasional interaction with patients experiencing confusion, delirium, or behavioral challenges
Physical Requirements (Combined)
This role requires the ability to:
Communicate clearly both verbally and in writing
Use manual dexterity for documentation tools (phone, tablet, computer)
Alternate between stationary positions and active movement throughout the day
Lift and carry up to 15 lbs while navigating stairs or uneven surfaces
Perform physical movements such as bending, kneeling, squatting, reaching, and grasping
Assist a patient (up to 250 lbs) from sitting to standing and back using appropriate body mechanics

In compliance with New Jersey’s Pay Transparency Act, the total compensation range for this position is USD $50,000 to $90,000. When determining an offer, VNA Health Group considers role responsibilities, experience, education, skills, internal equity, and market conditions.
