All positions with Santa Fe Public Schools require a post-offer/pre-employment background check. Some positions have additional requirements as established by federal or state rules and regulations. This position is funded through external grant sources and is contingent upon the availability of continued funding. Employment in this role is expected to continue as long as grant funds are secured; therefore, the position may be modified or discontinued if funding is reduced or no longer available.
General Function:
Provides coordinated and comprehensive case management to support teen parents and their children in accessing services, resources, and supports that reduce barriers to academic success and effective parenting. Promotes positive health, developmental, and educational outcomes for participating families. Collaborates with Santa Fe Public Schools GRADS program staff to support implementation of program goals, reinforce policies and procedures, and develop community partnerships that enhance program effectiveness and service delivery.
Education/Training/Experience:
● Bachelor’s degree in social work, counseling, education, human services, or a related field required;
● Minimum of two (2) years of experience in case management, social services, school-based support services, or a related field.
● Demonstrated experience in coordinating services and collaborating with community agencies
● Experience working with adolescent parents and/or at-risk youth populations preferred.
● Valid driver’s license and ability to travel locally, as required.
Knowledge,Skills and Abilities:
Knowledge of:
● Adolescent development, parenting education, and family systems.
● Community resources and social service systems that support young families.
● School systems, attendance requirements, and barriers impacting student success.
● Case management practices, including documentation and service coordination.
● Applicable confidentiality laws and ethical standards (e.g., FERPA).
Skills:
● Strong case management and organizational skills, including the ability to manage multiple students and families.
● Effective communication and interpersonal skills, with the ability to build trust and rapport with teens and families.
● Skilled in collaboration and partnership-building with school staff and community agencies.
● Proficiency in data entry, documentation, and record-keeping systems.
● Strong problem-solving and crisis response skills.

Essential Job Functions:
Performs all functions in accordance with established policies, procedures, safety and environmental regulations, facility and computer security policies and procedures. Functions are subject to change from time to time to meet the needs of the organization. Current essential functions include the following:
Student Engagement and Case Management
1) Demonstrate active listening skills while maintaining clear and appropriate professional boundaries.
2) Build trusting, professional relationships with students through consistent engagement and practical support.
3) Provide individualized navigation services aligned to student priorities, ensuring culturally responsive, team-based service delivery and documentation of cross-agency collaboration.
4) Meet regularly with students to monitor progress and assess or reassess needs.
5) Conduct home visits, as appropriate, to assess safety and support needs within the home environment.
6) Provide crisis intervention, support, and guidance to teen parents.
Academic and Skill Development Support
1) Provide academic support as needed.
2) Advocate for the educational needs of teen parents in collaboration with teachers and school staff.
3) Provide guidance in appointment management, time management, and balancing school and family responsibilities.
4) Support development of executive functioning skills, including goal setting, decision-making
Health Access and Family Support Services
1) Facilitate referrals and linkages to health-related and social service agencies through established processes
2) Ensure expectant students have access to prenatal care and related health services.
3) Assist in coordinating and scheduling health-related appointments for students.
4) Support students in accessing and enrolling in health insurance programs.
5) Assist eligible students with enrollment in public assistance programs, including WIC and SNAP.
6) Inform students of available community resources to support health, wellness, and family stability.
Collaboration and Community Partnerships
1) Develop and maintain collaborative relationships with school staff and multidisciplinary support teams.
2) Establish and sustain partnerships with community agencies to support coordinated service delivery.
3) Collaborate with community health navigators and partner organizations to streamline referrals and service delivery.
4) Work in partnership with teen father mentors and related support programs.
5) Collaborate with TPP staff on student case reviews, goal setting, identification of needs, and problem-solving strategies.
6) Participate in community coordination efforts, including monthly Santa Fe Connect meetings and Unite Us training.
Documentation, Compliance, and Reporting
1) Obtain and maintain required consent and release-of-information forms.
2) Document and track all referrals to internal and external resources in accordance with program requirements.
3) Utilize the Unite Us platform to document, monitor, and follow up on all referrals.
4) Collaborate in the development and ongoing maintenance of a local resource directory.
5) Prepare and submit timely quarterly and annual reports, including year-end summaries.
Professional Responsibilities
1) Attend and actively participate in required weekly program meetings.
2) Participate in ongoing professional development and training opportunities.
3) Stay informed on current research and best practices in early childhood development, education, and social services.
4) Perform other related duties as assigned.
Salary Range:
$50,000-$55,000
Resources Used in Performing Job Include (but are not limited to):
Standard educational and office equipment including computers, phones, copy machines, etc. Automobile to travel to school sites and/or meetings.
Physical/Cognitive Requirements:
On occasion required to lift children up to four years of age. Must possess and maintain a valid New Mexico Driver's License. The work requires the use of hands for simple grasping and fine manipulations. At times bending, squatting, reaching and the ability to lift, carry, and push up to 25 pounds. Proficiency with Google Suite.
Environmental Conditions:
Exposure to babies and potential related illnesses. Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. Some sitting, standing, bending and reaching may be required. Some occasional travel is required for meetings and conferences.
