Responsibilities:

  • Observe and document resident changes in condition accurately per policy

  • Ensure continuity of care by providing direct care and supervising licensed practical nurses, nursing assistants, and nursing students

  • Monitor, treat, and report resident pain utilizing pain management techniques

  • Administer medications following medication rights to reduce medication errors

  • Give thorough reports to oncoming shifts by documenting in resident records and 24-hour reports

  • Assist with medical procedures as needed

  • Maintain role as resident advocate with a focus on the mission of the facility

  • Implement plan of care based on assessments and goals established by the interdisciplinary care team

  • Ensure residents at end of life or on hospice services are kept clean, dry, and comfortable

  • Educate patients and family members on treatment and care plans, and answer their questions

Professional Requirements:

  • Current and valid RN License in New York State
