MDS/Care Plan Coordinator

Grandview Rehab is seeking an experienced and detail-oriented MDS Coordinator to join our Skilled Nursing Facility team in Connecticut. The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in compliance with federal and state regulations to ensure accurate clinical documentation, quality outcomes, and proper reimbursement.

Qualifications:
  • Active Connecticut nursing license in good standing (RN preferred; LPN considered with strong MDS experience).

  • Minimum 1–2 years of MDS Coordinator experience in a Skilled Nursing Facility preferred.

  • Strong knowledge of:

    • MDS 3.0

    • PDPM

    • Care planning process

    • Medicare documentation requirements

  • EMR experience.

  • Strong organizational, communication, and time management skills.



Responsibilities:
  • Coordinate and complete MDS assessments (including OBRA, PPS, and Medicare/Medicaid assessments) according to required timelines.

  • Ensure compliance with CMS regulations, CT Department of Public Health requirements, and facility policies.

  • Coordinate and lead the interdisciplinary care plan process.

  • Ensure accurate documentation to support PDPM and reimbursement integrity.

  • Collaborate with Nursing, Therapy, Social Services, Dietary, and other departments to obtain supporting documentation.

  • Maintain the MDS schedule/calendar and ensure all assessments are completed on time.

  • Participate in audits, QA/QAPI initiatives, and survey readiness.

  • Provide education and support to staff regarding documentation requirements.

  • Track and monitor Medicare Part A documentation and required skilled services.

  • Communicate with clinical and administrative leadership regarding compliance risks and improvement opportunities.
