Description
An Outreach Specialist is a professional who is responsible for working with patients and clinical staff to identify and remove barriers to care for patients and members of our community. They interact with all members in our community to assist them in addressing any social determinants of health and will participate in the community, by attending community events and fairs.
Requirements
- Requires a high school diploma or equivalent.
- Previous experience in related medical, dental, behavioral health community work is required.
- Individual must be professional and self-motivated, have strong attention to detail, good customer service and computer skills, knowledge of insurance plans and the ability to work independently and as a team.
- Must maintain Basic Life Support (BLS) and First Aid certifications for all life cycles.
- Must have mature judgement and good communication skills with ability to demonstrate cultural sensitivity.
- Must demonstrate excellent internal and external customer service skills.
- Must have reliable transportation and a valid driver's license.
- Provide coverage at other centers or events on short notice when needed.
- Participate in all Health Events and Late Nights.
- Certification as an Interpreter is preferred and ability to provide bilingual patient care is a plus.
- Must be able to fluently speak, read and write in English and Spanish and/or Creole.
Summary
- Access, review and address identified barriers to care through Social Determinants of Health (SDOH) questionnaire and uninsured patient list.
- Assist patients with completing and submitting initial and renewal applications for benefits or coverage such as Medicaid, Marketplace, Disability, Veterans Affairs, Snap, etc., documenting ALL assists, denials and approvals.
- Connect patients with available internal and external services.
- Schedule and document transportation for patients.
- Participate in a minimum of ten (10) community outreach events in addition to the Health Events & Center Late Nights.
- Document and follow up on each outreach visit, whether by phone or face to face, in the electronic health record.
- Other duties and tasks as directed by the Outreach Coordinator.
ADDITIONAL DUTIES ASSIGNED TO DESIGNATED BILINGUAL STAFF
1. If qualified, perform interpretation/translation services for ESRHS centers.
