Your work will focus on maximizing quality of life and movement potential through promotion, prevention, diagnosis, treatment, intervention, and rehabilitation. Guided by a physician-established Plan of Treatment and in collaboration with patients and caregivers, you’ll make a real difference in supporting healthier, more independent living.

Key Responsibilities:
- Follow and uphold established policies and procedures
- Improve or minimize residual physical disabilities for patients
- Help patients reach their optimum level of function and independence
- Collaborate with all healthcare personnel in patient care planning
- Take initial patient histories and conduct evaluations
- Perform skilled procedures as ordered by physicians
- Consult with physicians regarding changes in treatment
- Educate patients and families on home programs and activities of daily living
- Attend patient care conferences as scheduled
- Prepare clinical and progress notes
- Assist in developing and revising the plan of care as needed
- Communicate with families and agency personnel
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with agency timelines
Haven Qualification Standards:
- Current Occupational Therapy license in Florida
- Alignment with the philosophy and goals of Haven Home Health
- Ability to exercise initiative and independent judgment
- Strong clinical knowledge and expertise
- Skill in managing patients with diverse personalities
- Commitment to continuous learning and professional growth
