h/care was born in 2018 with a serendipitous meeting between two visionary healthcare entrepreneurs as they waited for their elevator in Birmingham, Alabama. United by a shared passion for transforming post-acute care, they spent thousands of hours visiting agencies, listening to caregivers, and gathering feedback from residents and their families. This journey culminated in the founding of h/care in 2023-a company built on a foundation of high touch, high tech, and unwavering trust, dedicated to delivering exceptional care and experiences for both our internal and external customers.

Your mission: Collaborate with occupational therapists to implement treatment plans, assist patients in regaining their ability to perform daily tasks, and provide education to families for continued care at home. You'll help bridge the gap between recovery and independence, ensuring patients feel supported every step of the way.

- Afternoon Check-Ins: Touch base with your therapy lead to report progress or seek guidance on challenging cases. Attend any virtual or in-person team meetings to align with organizational goals and patient care standards.
- Patient and Family Education: Educate patients and their families on how to continue therapeutic exercises, use adaptive equipment, and maintain progress between visits.
- End-of-Day Review: Reflect on the day's accomplishments, address any unresolved issues, and prepare for the next day's patient visits. Submit all completed documentation and communicate any urgent matters to the therapy lead or clinical supervisor.
