Job Description

Responsible for coordinating all aspects of nursing care for Memory Care residents.

ESSENTIAL DUTIES AND RESPONSIBILITIES 

Universal Philosophy

·         Upholds expectations for safety on the unit by being constantly aware of surroundings and resident needs.

·         Maintain constant interaction with residents to provide active engagement and customer focus.

·         Ability to redirect and engage with residents in a social and supportive manner.

·         Possesses the ability to provide creative interventions for resident social and clinical needs.

·         Stimulates and motivates residents as often as possible.

·         Operates under the universal worker philosophy.

Role Specific

·         Implements and coordinates care delivery collaboratively with physicians and resource health care personnel.

·         Provides care-related education and information to patients and families as appropriate.

·         Interacts with other departments to ensure optimum patient care.

·         Supervises other staff associates while on duty and provides training and coaching on all aspects of the Memory Care Program.

·         Ensures teamwork through the universal worker philosophy on the unit.

·         Takes ownership and responsibility for resident care, anticipates needs, and takes proactive measures.

·         Follows the physician’s order and medication administrating policy as it applies to the safe medication and treatment administration.

·         Appropriately documents medications, treatments, and other pertinent findings related to patient care.

·         Ensures that medications and treatments are completed on time.

·         Notes and transcribes physician orders for medications and treatments accurately and appropriately.

·         Gathers all pertinent information and completes incident report appropriately, notifying physician and responsible party.

·         Provides a high standard of care by auditing and ensuring compliance.

·         Addresses any scheduling conflicts or issues that arise, finding solutions to maintain required staffing levels and patient care standards.

·         All other duties as assigned.

 

SUPERVISORY RESPONSIBILITIES

This role is responsible for supervising CNAs.

 

QUALIFICATIONS EDUCATION and/or EXPERIENCE

To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

·         An Associate’s degree or equivalent from a two-year college or technical school, and 2 years related Memory Care experience (or equivalent combination of education and experience).

·         Effective written, verbal, and interpersonal communication skills.

·         Experience working with Electronic Medical Records (EMR) systems.

 

LANGUAGE SKILLS

Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public.  Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.

 

MATHEMATICAL SKILLS

Ability to work with mathematical concepts and to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

 

REASONING ABILITY

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

 

CERTIFICATES, LICENSES, REGISTRATIONS

Valid Florida LPN licensure, CPR.
