Job Description

The Community Service Board of Middle Georgia's Screven I/DD Day Services is seeking a qualified individual to serve as our next Direct Support Professional Team Lead. This position involves supporting individuals with developmental disabilities through a combination of direct care services, accurate documentation, and oversight of program operations. 

LOCATION: Sylvania, GA, Screven County (not a remote position)

Job Description/Duties:

  • Updates Personal Focus Worksheets yearly as assigned. 
  • Maintains individual records and update as needed/participate in audits. Complete HRST's as assigned and assist with SIS as needed. 
  • Completes Pre-ISP's and ISP's as assigned. 
  • Transports individuals safely to appointments or as assigned. 
  • Assists the CSW's as assigned and fill in as needed. 
  • Attends training as scheduled. 
  • Discusses individual needs with family as needed. 
  • Keeps up with physicals, PPD's, Flu shots, etc. 
  • Checks Therap records daily for accuracy and frequency of service; provides feedback to DSP's and provides them with documentation training as needed. Open and close building as needed. 
  • Assist the Director/Program Manager as assigned and serve as the Acting Program Manager in the absence of the Program Manager. 
  • Answer phones, Greet Visitors 
  • Assist with temperature checks as needed. 
  • Assist with interviews as needed or scheduled. 
  • T-Logs Daily/Monthly Seizure Logs
  • Assist in Kitchen as needed
  • Other duties as assigned.

Requirements/Preferences:

  • High school diploma, GED, or equivalent document.
    • Bachelor's degree in a behavioral health related field preferred but not required. 
  • Valid GA Driver's License, preferably someone 20-21+ years of age to drive company vehicles.
  • Pass a drug screen and background check.
  • Basic Computer Skills
