Position Summary

The Family Health Advocate at Mid-Columbia Children’s Council (MCCC) creates, provides, and coordinates services and activities with families and communities that foster strength, healthy living, and overall family well-being. They also provide case management to act as a liaison between families, staff, and the community. The Family Health Advocate provides support and education in the areas of positive parent-child relationships, families as lifelong educators and learners, program transitions, and connections to peers and the community. The Family Health Advocate encourages all family members to become advocates and leaders for their children and family.

Essential Duties and Responsibilities

The essential functions of this job include, but are not limited to:

 

CHILDREN & FAMILIES

1.     Develops positive and goal-oriented relationships with families.

2.     Assess family strengths and needs and provides resources to ensure family well-being.

3.     Provides three to four home visits per year, per family over the course of each program year.

4.     Provides parenting education on a variety of topics and modes that facilitate positive parent-child relationships.

5.     Recognizes and strengthens families in their role as the primary, lifelong educator of their children.

6.     Encourages and facilitates families in their role as children transition within MCCC classrooms or to other programs.

7.     Informs families of relevant opportunities and connects them with peers and the community.

8.     Assesses the educational needs of families and assists parents with individualized goal setting, providing guidance and resources to support families in achieving their goals.

9.     Plans, schedules, and facilitates parent meetings and parent training on a variety of topics.

10.Encourages families to become involved in the program and facilitates volunteer processes.

11.Monitors attendance and educates families on the importance of consistent attendance; creates attendance support plans and performs safety checks with families as needed.

HEALTH SERVICES

12.Determines each program participant’s health status upon enrollment and connects them with any needed resources. This may include, but is not limited to, insurance resources, medical, dental, and other healthcare providers. The FHA will assist program participants in these areas by providing reminders, scheduling assistance, and connecting them to transportation services as needed.  

13.Supports parents and participants to understand and complete physician recommendations, prescriptions, and referrals.

14.Ensures that health screenings are completed within specific program deadlines. Shares screening results with families and supports follows ups on all referrals as needed.

15.Facilitates the completion and implementation of medical protocols and food substitutions.

ADVOCACY

16.Advocates for children and families.

17.Commits to fostering diversity and inclusivity within the program’s environment.

18.Promotes the advocacy and leadership skills of families within MCCC and community partners.

19.Collaborates with teachers and other family support staff to ensure coordinated participation in the classroom, home, and community.

20.Maintains an active knowledge of community needs and resources.

ADMINISTRATIVE

21.Completes required reports, records, and case notes on each child. Enters data into appropriate software programs.

22.Assists parents with translation services as needed.

23.Participates in agency activities such as funders’ reviews, self-assessment, and all trainings.

24.Creates and maintains physical site files for the children on their caseload and ensures that all contact information and permission to release forms are updated and accurate.

25.Participates in recruitment activities to maintain full enrollment.

26.Completes the application and registration process for children accepted into the program.

OTHER

27.May provide coverage in the classroom. FHAs with a full caseload will not provide more than 10 hours per week of coverage, but FHAs who do not have a full caseload may be asked to provide more coverage than this if needed.

28.Reports all suspected child abuse and neglect as a mandatory reporter.

29.Performs all other work-related duties as assigned.

 

Minimum Qualifications

Required

·       Bilingual English/Spanish required, applications must be proficient in both writing and speaking both languages. 

High School Diploma or equivalent.

·        Related experience or training in a social services field.

·        Social Services Credential within 18 months of hire.

·        Valid driver’s license, proof of insurability, and cleared state driving record check.

 

Preferred

·        Associate’s or Bachelor's degree in Social Services or a related field of study.

·        Two years related experience working in the social services field, preferably with young children, adults, and low-income families.

·        Knowledge of community service agencies and resources.
