About the Role

As our Intake Coordinator & Office Manager, you'll be the first point of contact for incoming patients and referral sources — managing the full intake process from initial referral to successful admission. You'll also keep our office running smoothly, supporting our team with supply management, scheduling, and administrative coordination.

What You'll Do

  • Manage all aspects of the patient intake and referral process from start to finish
  • Coordinate seamless transitions to home care by communicating with patients, families, referral sources, and clinical staff
  • Perform insurance verification and obtain prior authorizations to ensure timely admission and maximum reimbursement
  • Establish and maintain strong relationships with referral sources and community partners
  • Ensure compliance with state, federal, and agency intake requirements for home health
  • Coordinate and process orders for durable medical equipment (DME) as needed for patient care
  • Route and communicate lab results to the appropriate clinical team members in a timely manner
  • Monitor and follow up on patients who are hospitalized, coordinating communication with the clinical team to support continuity of care
  • Assist with survey preparation and support agency readiness for regulatory reviews
  • Answer and direct incoming calls in a professional and caring manner
  • Manage office inventory, order supplies, and maintain an organized work environment
  • Assist other office staff as needed to support smooth day-to-day operations
  • Assist with onboarding new employees as needed
  • Identify opportunities to improve intake workflows and patient experience
  • Schedule on-call weekends
What We're Looking For

  • Prior experience in a healthcare setting — home health experience strongly preferred
  • Experience with insurance verification and prior authorization processes preferred
  • Strong organizational skills and exceptional attention to detail
  • Excellent verbal and written communication skills
  • Ability to multitask and remain calm in a dynamic environment
  • A compassionate, team-oriented attitude with a genuine commitment to patient care
Compensation & Benefits

  • $26-$29 per hour, based on experience
