Salary Range: £57,635 - £68,571 or BAR £70,898 - £75,789 per annum

   

Working Arrangements: 37.5 hours per week

 



About the Role:

The States of Guernsey are looking for an Occupational Therapist to join the Rapid Response and Reablement Team.

This role serves as the Occupational Therapist responsible for the operational, timely assessment, planning, implementation and evaluation of care and rehabilitation to service users who have been referred to the Rapid Response and Reablement Team. This includes supporting frail, elderly individuals and those who are acutely unwell, with the aim of promoting quality of life, independence, and personal choice wherever possible. To take a lead role in developing high standards of current evidence-based practice and leading occupational therapy services. To have an active role in education and training of staff working within the Rapid Response and Reablement team.

To commit to work collaboratively within multidisciplinary teams (MDT’s) maintaining strong communication with colleagues across services such as the emergency department (ED), inpatient wards, admission avoidance, and community providers including, community carers and district nursing teams to provide a high standard of service in a variety of settings including service users’ own homes, community settings and long-term care facilities. Providing high standards of MDT working to enhance service delivery and improve patient outcomes.
