Description

Job Posting: Home Health Aide (HHA)


Job Summary:
We are looking for a compassionate and skilled Home Health Aide (HHA) to join our team. In this role, you will provide personal care services to patients in their homes under the supervision and direction of a Registered Nurse (RN). You will play a vital role in supporting patients’ health and well-being by assisting with daily activities and ensuring the home environment is safe and comfortable.


Responsibilities:

  1. Adhere to established Agency policies and procedures.
  2. Provide personal care, including bathing and hands-on care, as assigned.
  3. Complete visit records promptly and accurately, as per Agency policy.
  4. Report any changes in the patient’s condition or needs to the RN.
  5. Perform household services essential to health care in the home, as directed.
  6. Assist with ambulation and exercises as per patient care plan.
  7. Perform simple procedures, such as range of motion (ROM) exercises, under the guidance of the RN or therapist.
  8. Assist patients with medications that are ordinarily self-administered, as assigned.
  9. Attend scheduled inservice and continuing education programs.
  10. Participate in patient care conferences to discuss care plans and patient progress.

Working Environment:

  • Works indoors in Agency office and patient homes. Travel to/from patient homes is required.

Job Relationships:

  • Supervised by: Director of Clinical Services/Clinical Manager/RNs, PTs, OTs, SLPs
  • Workers Supervised: None

Risk Exposure:

  • High risk

Lifting Requirements:

  • Ability to participate in physical activity, including heavy lifting, bending, and standing for extended periods.
  • Ability to perform tasks involving lifting and assisting patients in daily activities.
Requirements

Qualifications:

  • Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
  • A sympathetic attitude toward the care of the sick and elderly.
  • Ability to follow instructions, read, and write.
  • Maturity and ability to handle the emotional and physical demands of the job.
