This is a full-time in person position.

You MUST be able to work some weekends, remote.

**Bilingual in Russian preferred.

Pay: $21.00-$24.00 per hour

Job Summary: The Home Care Coordinator is responsible for coordinating and scheduling patient care. The Home Care Coordinator is responsible for coordinating and overseeing home care services to ensure clients receive timely, high-quality, and compliant care. This role serves as a liaison between clients, caregivers, clinical staff, and external partners, ensuring care plans are implemented effectively and services meet regulatory and organizational standards.
**Bilingual in Russian preferred.

Responsibilities:

  • Coordinate the scheduling and ongoing management of home care services.

  • Match clients with appropriate caregivers based on home care needs, availability, and skill set.

  • Communicate regularly with clients, families, caregivers, and clinical staff.

  • Monitor service delivery, address issues, and ensure continuity of care.

  • Maintain accurate documentation and ensure compliance with state and agency regulations.

  • Respond to client concerns and resolve service-related issues promptly.

  • Participate in quality assurance and performance improvement activities.

  • Coordination with patients, families and insurance contacts to ensure services are scheduled and plans and any changes are communicated promptly and effectively.

  • Manage scheduling of a patient caseload and associated responsibilities, such as pre-billing, attendance, and visit verification.

  • Accurate and timely documentation in patients' and caregivers' records as applicable.

  • Ability to work occasionally on weekends and holidays as needed.

  • Identify and escalate issues to supervisors.

  • Perform other duties as assigned.

Qualifications:

  • Strong communication, skill, both written and verbal.

  • Excellent customer service skills.

  • Strong time management and organization skills.

  • Adaptability and flexibility.

  • Bilingual Spanish preferred.

  • Proficient in Microsoft 365.

  • HHAX experience preferred.

  • 1 to 3 years' experience as a Home Care Coordinator.

  • Ability to multitask.
