Job Summary
Serves as a Medical Social Worker across Home Health and Hospice. Promotes and maintains high-quality mental health and psychosocial care for patients and families. Establishes plans of care, conducts monthly visits, provides crisis intervention, discharge planning, therapy services, and community referrals while working collaboratively within a multidisciplinary treatment team and ensuring full regulatory compliance.
Minimum Qualifications
• Master’s Degree in Social Work (MSW) from a Council on Social Work Education-accredited program.
• Current licensure by the State of Mississippi.
• Board certified, credentialed, or eligible for credentialing in social work.
• Minimum of two years supervised experience in healthcare or related multidisciplinary setting.
• One year experience in a psychiatric setting or social service agency.
Preferred Qualifications
• Experience with Medicaid and Medicare payment systems.
• Prior experience in Home Health, Hospice, or Behavioral Health.
Essential Duties & Responsibilities
• Establish, implement, and manage individualized plans of care for patients and families.
• Perform biopsychosocial assessments, suicide risk evaluations, and crisis intervention counseling.
• Provide individual, family, group, and marital therapy services as indicated.
• Facilitate discharge planning and community referrals to support continuity of care.
• Participate as an integral member of the interdisciplinary treatment team.
• Organize and facilitate individualized treatment plans and reviews.
• Provide on-call services to the Emergency Department for behavioral health evaluations.
• Consult with physicians, legal authorities, community agencies, and referral sources.
• Maintain accurate, timely documentation per facility policy and regulatory standards.
• Participate in department meetings, case reviews, quality assurance, and performance improvement activities.
• Educate patients, families, and community groups regarding available resources and treatment options.
• Maintain patient confidentiality and adhere to all policies, procedures, and bylaws.
• Attend required in-services, continuing education, and maintain CEUs for licensure.
Clinical Responsibilities
• Conduct psychosocial histories and family interviews.
• Develop comprehensive, individualized treatment plans and master problem lists.
• Document progress toward treatment goals and recommend revisions when appropriate.
• Provide primary therapy services for Behavioral Health Clinic patients.
• Coordinate follow-up care and community resources to ensure continuity of care.
Professional Responsibilities
• Demonstrate flexibility and time-management skills during peak patient volumes.
• Adjust work schedule as needed to meet departmental demands.
• Participate in staff meetings, case conferences, and community education activities.
• Complete competency-based evaluations and maintain professional development.
Physical Requirements
• Ability to stand, walk, sit, bend, stoop, kneel, crouch, twist, and balance as needed.
• Ability to push, pull, lift, and carry up to 35 pounds intermittently.
• Frequent use of hands and fingers for documentation and therapeutic activities.
• Adequate visual, auditory, and verbal abilities to communicate effectively with patients, families, physicians, and staff.
Working Conditions
Provides care in clinical, hospital, home health, hospice, and emergency department settings. Exposure may include emotionally distressed patients, mental health crises, and varying environmental conditions. Requires collaboration with multiple departments and external agencies while ensuring compliance with CMS and other regulatory standards.
